Recently, while testing an old flow, the error "Action 'Create_BOQ_Log_Record' failed: A record with matching key values already exists" occurred. The issue was in the "Create BOQ Log record" step, where the Primary Key Field/GUID field of the BOQ Log record was being set with the GUID of the Parent BOM record. This caused a duplicate key exception when trying to create a new record with the same parent's GUID. Clearing the field and letting the system generate the GUID resolved the issue.
